Picture Day Procedure

Please print out the order form ( full size )

Fill out your form prior to picture day and bring it with you to your scheduled time for pictures. ( Do not forget sales tax. Total x .0625 )

It is VERY important to be on time in order for your child to have his/her picture taken with the team. We ask that you have your child in the team picture even if you’re not purchasing pictures so that the players that are buying pictures have a complete team photo.

 Please bring your order form with you and check in at our table

Our staff will check you in, collect your payment, (cash, credit card or check ), and stamp your order form PAID, and will then give it back to you.

 Your child must have this form in their hands when they step up to have their individual picture taken.

 Coaches and players only are allowed in the picture area

Cameras are not allowed (including cell phone cameras )

Please write legible.  We will not reprint orders if your writing is illegible.
